The Service Coordination Certificate is required for all Early Interventionists who will be functioning as service coordinators for families. REQUIREMENTS – All requirements MUST be completed PRIOR TO the one day training:1) MUST have previously completed all online modules, exam and received the Initial Birth to Three Certificate. 2) MUST have previously completed all online Service Coordination modules and passed the online service coordination exam. 3) MUST have minimally completed 3 required observations at your program: Initial Evaluation, Initial IFSP, IFSP Review or Annual. Information on modules is available HERE: http://www.birth23.org/providers/pd/rtsct-2/ •PLEASE check the previous link for information BEFORE emailing Birth to Three with questions. 4) MUST Upload and submit Initial Certificate, Service Coordination Module Completion Certificate, and Attestation of Observation form through Protraxx in the CT Birth to Three-Service Coordination Prerequisites Class. All 3 assignments must be received by the deadline. No later than one week prior to training. Certificates received after this date can be retained for the next training date. Thanks!
